Medical rehabilitation in Germany: immersion at the heart of a system in transformation

Organized in partnership with ANAP, this study visit brought together a French delegation around a shared objective: to understand how the German system structures and optimizes medical rehabilitation services, particularly in a context of evolving care pathways and the expansion of ambulatory care.

From March 9 to 11, 2026, Dialog Health, in partnership with ANAP, supported a delegation of healthcare professionals during a study visit focused on medical rehabilitation (SMR) in Germany.
Held in Berlin, this study visit aimed to provide a concrete understanding of organizational models, patient pathways, and innovation dynamics within a system recognized for its structure and adaptability.
The program was designed as a progressive journey, with each visit shedding light on a different dimension of the system.

ZAR Berlin – Zentrum für ambulante Rehabilitation
Outpatient rehabilitation clinic. Orthopedics, neurology, psychosomatic care, uro-oncology. Integrated telemedicine.
Located in the heart of the city, this center delivers rehabilitation without hospitalization. Patients arrive in the morning, follow a structured sequence of therapies, and return home at the end of the day.
An intensive, well-organized model, almost paced like a working day — offering a glimpse of what rehabilitation in urban settings could become.

MEDIAN Klinik Berlin-Kladow
Neurological rehabilitation center. Stroke, brain injuries, from early to post-acute phases.
Here, neurology sets a different pace. Care pathways are structured with precision, step by step, from early rehabilitation through to the gradual recovery of autonomy. Multidisciplinary teams operate with a high level of coordination, addressing often complex clinical situations.

Vivantes Rehabilitation – Auguste-Viktoria
Outpatient rehabilitation. Cardiology, neurology, oncology, orthopedics. Day hospital, multidisciplinary approach.
This outpatient center stands out for its capacity and organization. Patient flows are high, pathways are well coordinated, and the whole system operates with an almost industrial efficiency—while consistently maintaining its primary objective: restoring functional capacity.

Rehabilitation in neurology, orthopedics, and internal medicine. Located by Lake Tegel, with an interdisciplinary approach.

Here, rehabilitation follows a more measured pace. The natural environment is fully integrated into the care process. Recovery is not only physical, but also about quality of life, balance, and rebuilding.
Throughout the visits and discussions, several key themes emerge.

First, a deliberate diversity of models: inpatient care, outpatient care, digital follow-up, and hybrid combinations. The system adapts to patients, rather than the other way around.
Second, a highly structured pathway approach. Rehabilitation is embedded in a continuum, closely connected with other healthcare providers.

Finally, a strong capacity for practical innovation: telemonitoring, personalized programs, and optimized patient flow management—all reflecting a clear intention to improve efficiency without losing the human dimension.

With more than 1,600 rehabilitation centers and millions of patients treated each year, Germany confirms the central role of rehabilitation within its healthcare system.

Observing these transformations at an international level not only provides perspective, but also opens up concrete avenues for evolving practices.
